Left to cherish her memory is daughter Melissa Dvorak, granddaughter Nicole Dvorak, son Daniel (Linda) Coley, grandson Jackson Coley, daughter Leah Coley, and granddaughter Jazmine (Jordon) Coutu and four great-grandchildren.
Funeral service will be held at 2:00 p.m. on Thursday, September 16, 2021 at Chapel Lawn Funeral Home, 4000 Portage. Please RSVP to Chapel Lawn between 9:00 a.m. and 4:00 p.m. as space is limited.
